(I ran into this about two weeks ago, and posted.)  The problem is that
version 1.47 of the stylesheets and FOP don't get along well.  FOP is based
on an older version of the XSL-FO spec.  However other tools FO-renderers
support more up-to-date versions of the spec.  Now the stylesheets should
and are follow the tools' capabilites.  Unfortunately for FOP users, this
means that we're stuck with 1.46, until it is upgraded to a later version
of the spec.  (Norm indicated that the FOP folks said "in a few months").

If you "downgrade" to version 1.46 of the stylesheets, your problem should
go away.  Norm Walsh had talked about writing a stylesheet which would
transform 1.47 output into something that FOP could handle, but I don't
know if he ever got around to doing it.  (I'm running with 1.46 until I
have time to look at it.)
