Yes, this looks like a bug. The relevant test in
fo/refentry.xml is:
<xsl:template match="refentry">
...
<xsl:choose>
<xsl:when test="not(parent::*) or parent::reference">
<!-- make a page sequence -->
<fo:page-sequence hyphenate="{$hyphenate}"
This says to start a page sequence
if the refentry is the root element,
or if it is a child of <reference>.
But that should be "not(parent::reference)"
because a reference element already started a page
sequence.
