This might sound like an obvious question, but did you change "book" to 
"set" in the document declaration, like this:

<!DOCTYPE set PUBLIC "-//OASIS//DTD DocBook V4.1//EN"
           ^^^
We have been publishing our books in a set for over a year now with pretty 
good results.  We're still going the SGML route, but if you want to see how 
we do it, you're welcome to take a look at our in-house manual:
