Are you using a customization?  The 'callout.graphics'
parameter is set to 1 by default in the XSL stylesheets.

For fo print output, you might want to consider using
'callout.unicode' instead of 'callout.graphics'.
The unicode characters look the same, but are
higher resolution than the .png bitmap graphics.
