You can also import an external XML file using XInclude with a parse="text"
attribute, which escapes all the markup. Then you can use an areaspec to
place the callouts. It is tricky, because you have to use coordinates in
the areaspec, but you don't have to manually escape any XML code. You have
to use a Java processor to get the callouts extension, and you have to
configure in a recent version of Xerces (like 2.9.0) as the parser that can
handle XIncludes.
