Hi,
From your description, it sounds like you don't really want a blank page,
but a page break after the title. It also sounds like you don't want this
for all appendices, just the one with the form. You can use a processing
instruction along with a small customization to do that, as described here:

My appendix pages are arranged as follows:
The Appendix title (e.g., Appendix A. Application Form) appears
on a page by itself, and is followed by one or more pages.
When printing double-sided, the form will be printed on the
other side of the page with "Appendix A". How can I automatically
add a blank page after "Appendix A" to prevent this?
