Just a comment: I have been using SVG graphics with DocBook4 for years, but using DocBook5-style inclusion via imagedata (and customized RelaxNG validation). With this notation, the recent docbook-xsl stylesheets beautifully work out of the box: If your SVG root element contains a viewBox and width and height attributes set to 100%, then the resulting xhtml/firefox and fo/fop behave consistently with raster images.
So, the most parsimonious solution might be to fix this within the DocBook4 spec instead of (or in addition to) the stylesheets: Allow DocBook5-style SVG graphics inclusion in DocBook4 (in addition to the current inclusion as a direct child of imageobject).
