I think you need to be a little more specific about what happens, and about your setup. When you say "I have lots of missing elements", what exactly do you mean? Do you get error messages about templates that don't match? Do you use DocBook 4.X or 5.0? Do you use the namespaced or non-namespaced version of the 1.74.0 stylesheets? What XSLT processor do you use? A short test document that can be used to reproduce the problem could also be helpful.
