Are you using FOP? I didn't think this was supported:

The footnoteref is there, superscripted. The reference is to the
content
<sup>[<a name="d0e3227" href="#ftn.d0e3227" class="footnote">2</a>]</
sup>
But there is no actual footnote?
Am I missing something please?
