[docbook-apps] No element mapping definition found for fox:destination

I'm using DocBook XML v. 4.5, docbook-xsl.1.76.1, and FOP 0.93 and got the 
following error: "No element mapping definition found for fox:destination."

Not sure why I'm getting this error since I've set the following parameters my 
custom stylesheet:

        <xsl:param name="fop1.extensions">1</xsl:param>         <!-- 0 = False, 
non-zero=True -->
        <xsl:param name="fop.extensions">0</xsl:param>  <!-- 0 = False, 
non-zero=True -->

Any help would be appreciated.

(Everything worked fine in docbook-xsl.1.73.2.)
