pgwide is permitted on table, both CALS and HTML tables. Here is the definitive

I looked in the DTD and the RelaxNG schema, and see pgwide available on table. How
did you conclude that it was not permitted?

I've successfully used pgwide="1" on some wide tables with FOP.
However that was on a book using Docbook 4.x. I see with some dismay that Docbook 5
doesn't permit pgwide on tables... What replaced it?
