This was reported earlier, and the fix for this problem has been checked into the
latest SVN version of epub/docbook.xsl, so it will be in the next distribution.

The stylesheet epub/docbook.xsl (in ns-1.76.1) uses a "bookinfo" element in
the lines 776, 777, 1069 and 1070 - is this intended? I had to change it to
"info" for my DocBook 5 XML files (otherwise index.html is not referenced in
the manifest section and an itemref with a non-existing idref is used in the
spine).
