This could be done by formatting each index entry using fo:table and
using fo:retrieve-table-marker to show continuation text. This is XSL-FO
1.1 feature, I'm not sure which processor support it directly, but many
processor offer similar proprietary extension, for example there is
rx:table-omit-initial-header in XEP.
