Hi Dick,
Yes, that's a bug. The template was checking for the existence of
titleabbrev, not info/titleabbrev, before using mode="titleabbrev.markup",
but that check was unnecessary. That mode uses either titleabbrev if
available, otherwise it falls back to title. I made the small fix in SVN,
so it should appear in the next build.

I'm using the index.prefer.titleabbrev option, which uses <titleabbrev>
instead of <title> when building the links in the index. I'm using the
epub3 styesheets, but it looks like the index processing happens in the
xhtml stylesheets.
Everything works fine as long as <title> and <titleabbrev> are not inside
<info>. If they are inside <info>, then <title> is used instead of
<titleabbrev>.
If that looks like a bug, I can file an RFE, but I thought I'd check with
the group, first, to make sure I didn't miss something.
