Hi Dick,
Easier than you imagine. The index element supports title and block
elements like a chapter. If there are no indexentry elements, then the
stylesheet will built it automatically after your intro text. Something
like:
<index>
<para>My intro goes here.</para>
</index>

As long as I'm harping on indexes, is there an easy way to get
introductory text into the beginning of a generated index? I'd like to
have a paragraph that describes typographical conventions in the index, or
for another use case, be able to introduce a specialized index.
I could do this by hand, after the index is generated, but if there is a
better, cleaner way to do this, it would be nice.
