Hi,
Since it there until yesterday, that indicates that the system was
working then. Something must have changed in how you process the file
to cause the index to not be generated now. Have you investigated what
might have changed?

xsltproc /usr/share/xml/docbook/stylesheet/docbook-xsl/htmlhelp/
htmlhelp.xsl test.xml
My index.hhk file always looks like this.
