Declare the namespace it in your root element:

  <xsl:template version="1.0"
       xmlns:xsl="http://www.w3.org/1999/XSL/Transform";
       xmlns:d="http://docbook.org/ns/docbook";>

Add the prefix "d" before your element as in "d:glossterm". Remember,
you can choose whatever prefix you like ("d", "db", "docbook", ...).
Usually it more convenient to have a short one. What you can't change is
the namespace URI.
