This could potentially have something to do with a change in behavior in
FOP 2.0 (but I could only se part of this thread so not quite sure
whichi version of FOP you are discussing). I submitted this as a bug
last year to FOP. See https://issues.apache.org/jira/browse/FOP-2523
This has broken a number of my documnts when I moved to FOP 2.0
Currently I have "fixed" this by reverting back to FOP 1.1 for my PDF
processing since I simply didn't have more time to experiment until I
found a solution with FOP 2.0
