I'm looking at to add an confidentiality agreement to one of my docbook
books and I'm not sure of exactly how to go about it. The way it is
layed out in our non-docbook format is like:

AGREED AND ACCEPTED BY: 

       Date: _______________ 

  Agreed By: __________________________ Witness: ___________________________ 
      Title: __________________________

Accepted By: __________________________ 
      Title: __________________________


What would be the proper way to lay this out? How should I properly
handle the signature lines?
