Hello,
The <indexterm> tags just mark the targets of the links generated by the
DocBook stylesheets in the Index chapter. The Index chapter is generated
at the position of the <index> tag. Just insert a tag
<index/>
in your DocBook document where you want to have the Index chapter, for
example before the Appendix chapter.

Hello,
I've reviewed the documentation and some of the oreilly resources, and
I haven't been able to generate any of the index files that docbook
should make when it encounters <indexterm> tags.
Any recommendations or resources I should be reviewing that can help
me solve this issue?
I'm not passing any special commands to the docbook "compiler" and I'm
not sure if that's something I should be doing - much less what those
commands are.
