I created an eight-liner (DocBook 4.3 article) with a couple of umlauts and 
translated this document with the following command line:

xsltproc /usr/share/sgml/docbook/xsl-stylesheets/html/docbook.xsl
refdbtest.xml > refdbtest.html

There are no character entities. The generated HTML file contains the proper 
umlauts which are also displayed properly in a web browser.

Do you use any driver files that might contain weird modifications?
