Right now, I'm specifically focussing on problems with xsltproc. In no 
particular order:

1) Despite the man page's assertions, I get the error: Unknown option --docbook

2) I seem unable to get the --catalogs option working. The error is:
    warning: failed to load external entity "--catalogs"
    unable to parse --catalogs

    I have been trying various ways to get the $SGML_CATALOG_FILES environment 
variable set 
    correctly for the --catalogs option to work, but no luck.

3) The --xinclude option gives me the exact same error. I am (perhaps 
erroneously) assuming this option is used to load the xinclude.mod file in some 
docs directories. When I get a spare cycle or two, I'll look at the URL 
mentioned in the man page.

And that's where I'm at right now. I suspect that going through the entire XML 
book online may answer a few (all?) of these questions--Google's gazillion hits 
hasn't help with the SGML_CATALOG_FILES option so far--but I simply don't know 
when I'll get time to wander through the tome. My current experiments are 
twenty minutes here, a half-hour there. If I could find a few solid hours to 
focus... :-(  It doesn't help that I can only do this at home. The odd spare 
cycle at work doesn't help.

While I am able to get decent-looking PDF's generated, I'd like to be able to 
do so without all the errors, which imply that the PDF isn't fully conforming 
to the formatting, etc. laid out in the XML files. (Maybe this also explains 
the lack of page numbers in the index?)
