I have solved my problems.

Linux -> go through this: https://github.com/intellectsoft-uk/MssqlBundle 
and make sure you have the FreeTDS package installed and configured 
correctly.

Windows -> Even though my sql server was at the default port of 1433, you 
have to make sure you aren't sending the port in the dsn for some reason. 
Since I'm using symfony2, I commented out database_port: in parameters.yml 
and port: in config.yml.
