I think the problem is easily solvable by included the FK entity name in
the index name, so it may indeed be done via naming strategy (though it
probably won't work directly, as the naming strategy has no access to the
table schema object).

If you start from an independent test case and work from there, it should
be easy to get to some sort of resolution (or limitation, depending on what
we can do).

Submitting a Pull Request even just with the failing test case helps us a
lot!
