I downloaded a copy of Chapter 6, Impress. I opened it in
LibreOffice 5.0 RC1. (I use the Debian version[Linux].) I did not have
any problems with it creating an error message on the page where you had
inserted a comment. If you are using Windows, that might be the problem.
I checked how it was anchored: anchored as a character.
