I've now checked, and it appears that I was misremembering; the website link does not appear to pick up the file from the wiki.
As for the lack of sidebar bookmarks, I realise that I never remember to tick that box on the PDF Export dialog. I'll put that on my checklist for future books. Sorry, and thank you.
