BACTERIAL PATTERN AND SENSITIVITY TEST ON MALIGNANT CHRONIC SUPURATIVE OTITIS 
MEDIA

ABSTRACT

 

Background: The chronic suppuratif otitis media is a chronic middle ear 
infection producing discharge continuously or intermittenly, the perforating 
tympanic membrane and normally with hearing disorder. It is devided into two 
types there are: 1) the benign type limited to mucous, normally doesn,t affect 
the bone, seldom inducing dangerous complication without cholesteatoma; 2) the 
malignant type to have passed throught the periosteum pertaining to the bone 
invasion so that the bone destruction occurs, any cholesteatoma and often 
inducing dangerous complication. The incidence rate of otogenic intracranial 
complication in pre antibiotic period is compared after using antibiotic 
showing the decrease tendency. Antibiotic is one of the medicamentosa to have 
been used for the chronic suppuratif otitis media treatment, however so far 
there has been any controversi conserning the use of antibiotic, both topically 
and sistematically. However there have been many expert suggesting the
 administration of sistemic antibiotic in the chronic suppuratif otitis media 
treatment. The sistemic antibiotic administration in the chronic suppuratif 
otitis media treatment is often reported not to give the satisfied result. The 
success of treatment of the bacterial infectious disease with antibiotic is the 
outcome of the interaction of three components such as the patient, bacteria 
and antibiotic. The causal factors of the failure of the bacterial infectious 
diseases treatment using antibiotic namely : 1) the patient : the antibiotic 
doens,t reach the side of infection, the fungtional disorder of eliminating 
organ or being incomplete so that the drug becomes toxic, being allergic toward 
antibiotic used; 2) the bacteria: the misused of the etiological diagnosis, the 
resistensi of the bacteria causing the infection toward antibiotic 
administered, any superinfection; 3) the drugs : for example the incorrect 
choise of drug. 

Objective : To determine the bacterial pattern and the result of the 
sensitivity of bacteria that were bred from ear discharge of the patient with 
the malignant type of  chronic suppurative otitis media towards antibiotic. 

Study design : The retrospective research from 2003 to April 2005 with cross 
sectional data collection using the analysis of data taken from medical records 
of the patient with the malignant type of chronic suppurative otitis media. 

Material and method :  The collection of data was performed from medical 
records of the patient with the malignant type of chronic suppurative otitits 
media coming for a treatment at ENT policlinic of Dr. Sardjito Hospital of 
Yogyakarta within the period between 2003 and April 2005 and the analysis of 
data were performed. 

Result : Some kinds of bacteria could be identified from the result of culture 
namely Pseudomonas aeruginosa, Staphylococcus epidermidis, Staphylococcus 
aereus, Klebsiella sp, Streptococcus sp, Proteus sp, Enterobacter sp, 
Staphylococcus haemoliticus, E. Coli, and Bacillus with the most percentages 
were Pseudomonas aeruginosa. The result of sensitivity test Pseudomonas 
aeruginosa and Staphylococcus epidermidis has still been sensitive toward 
cefotaksim, klindamisin, ciprofloksasin, gentamisin and amikasin.

Conclusion : in conclusion, there is no dislocation of the pattern of the the 
chronic suppuratif otitis media causing bacteria within the period of  1982 to 
April 2005, in which the bacterial pattern shows Pseudomonas sp, Staphylococcus 
epidermidis and  Staphylococcus aereus as three types of dominant bacteria. In 
this study the resistensi of  Staphylococcus aureus ampisilin and amoksisilin 
25% is found. The result of sensitivity test of Pseudomonas sp and 
Staphylococcus epidermidis has still been sensitive toward imipenem, 
gentamycin, ciprofloxacin, fosfomicin and klindamisin.
