User has a huge mailbox, probably the client runs into a timeout and
disconnects.

No, the user logged in normally using LOGOUT command (cmd_logout() is called). Hmm. Maybe the client is trying to pipeline commands and issues a logout while another command is still running. Any idea what the client is?
