Another thing i noticed that you are using 

login_access_sockets = tcpwrap

I wonder if tcpwrap might be interfering here?

Also, since you are using FreeBSD, can you use sockstat -l to determine if 4190 
is listed there anymore?

Did you check that you haven't firewalled the port off?
