I haven't been able to reproduce this with a simple example. So I suggest to 
try with a 
small example without any customisations to the header/footer/stylesheet first.

index.hhp does indeed not include the .js file, but this is not needed as
the HTMLHelp compiler will find them (you can use hhw.exe to decompile the .chm 
to verify this)
