Hello,

I have recently upgraded my version of Doxygen from 1.3.5 to 1.8.0. We use 
Doxygen to generate a .chm file using the HTML Help Compiler.

Since we have upgraded I have noticed that the table of context being generated 
contains all of the related pages we have added, directly under the table of 
contents. Previously the related pages used to be grouped under a root filter 
page called "Related pages", and that way the table of contents was more 
organised and not spammed.

Also previously if a related page had a section, the section was never being 
displayed under the page in the table of contents. What I am finding is that 
the even the section is now being shown in the table of contents, and it is 
shown under the page with a book icon.

It would be very nice to sort these three issues out


1.       Group all the related pages under a single filter when showing them in 
the table of contents

2.       Fix the icons of pages so that the icon is a page and not a book

3.       Do not show sections, subsection and subsubsections in the table of 
context.
