I regenerated using 1.8.3.1 and I have a different set of problems: 1) The included text is still numbered but without the leading zeroes. This is still wrong, as far as I can tell, but now it looks even worse.
2) The included text is supposed to appear rendered as if it was surrounded by \code and \endcode.
