Currently the Fortran interpreter in doxygen converts everything to lower case (as you noted), this is done as Fortran is a case insensitive language and otherwise it would be quite hard to link the different items within the Fortran part with each other (developers of Fortran code don't always stick to a fixed case convention in their code). There are plans to keep the case as used in the source code, but it is quite a big task to set everything to the right case, so this will happen when time permits.
