Summary: Fatal error of a user does not remove them from session

Initial Comment:
If someone is forced offline by serious/fatal error (network disconnect, their 
Eclipse crashes etc.), he is not removed from the session. As a consequence he 
can't be reinvited (because he is still in session).

To reproduce:
Alice and Bob are in a session.
Bob suffers fatal error that cannot close session cleanly (disconnect from 
network, or kill Eclipse)
Bob signs back into Roster.
Alice sees that Bob is still in the session, so cannot invite Bob.

Yes, it works, but only if Bob stays offline for the entire 60s. If Bob
comes back online earlier, Alice believes that Bob is still in the session.
There is no "I'm not in your session anymore, stop bothering me with your
activities" message (activity) in Saros.
