Steven, Make sure that the path in the .reg file has double backslashes for the path delimiters. A good way to make sure the path in the registry is correct is to merge the .reg file to the registry (double-click it), then edit the registry directly with regedit to set the path.
Attached is a screenshot of what my registry entry looks like. Export the registry key HKEY_CURRENT_USER\Software\Microsoft\Internet Explorer\MenuExt, and send it to me directly (glenn at glenncarr dot com), and I'll see if I see any problems.
