You don't need a transformer....

Wire color codes vary, but _usually_ microphones are wired so that
the shield goes to the "sleeve" of the connector, white goes to the
"tip" (PTT switch), and red goes to the "ring" (microphone).

Schematics are at:

http://bama.sbc.edu/astatic.htm

Easiest way is to connect one lead of an ohmmeter to the shield, the
other lead to the white wire, and push the PTT bar on the microphone.
If the meter goes from open to short, that lead goes to the "tip". If
the meter stays "open" when you push the PTT, or reads some value of
resistance, then that lead goes to the "ring".

If your D-104 has an amplifier in the base, it will work also. Same
connections as above.

You do NOT need a transformer for either case.
