It's not clear which level of abstraction could be causing this. Matt Emerson and Gail Zacharais have checked out the CCL level (what's going on inside with-output-to-stream), but it doesn't seem to be happening there. In fact I just don't see any way that this could be happening. I was hoping against hope that someone might have already seen this.
