After some additional digging, the clipping I am seeing is actually an artifact
of incorrectly rendered fog. 

I believe the commit I mentioned before along with the later patch from bug
9686 both contribute to my issue.

When I disable the _mesa_append_fog_code() call, the lighting appears normal,
but I still get the incorrectly rendered fog/haze in the distance (this appears
as a solid fill rendered rather than the semi transparant it should be and
therefore looks like the screen is clipped). I believe that the solution to bug
9686 is either incomplete or incorrect when coupled with the implementation of
_mesa_append_fog_code().
