FULL REVISION COMMENTARY:

2131.9.4: Stewart Smith 2011-02-04 remove ER_ERROR_LAST as it is completely
unused.
2131.9.3: Stewart Smith 2011-02-03 [merge] merge trunk
2131.9.2: Stewart Smith 2011-02-03 add test for 674719: START TRANSACTION
causes implicit COMMIT
2131.9.1: Stewart Smith 2011-02-03 on START TRANSACTION being called during a
transaction, output a warning that a transaction is already in progress. This
is the same behaviour as PostgreSQL. We used to do an implicit commit. This is
totally not the right thing to do.
