Hi,

Please make sure that whatever metadata you have added in the 
input-forms.xml file, it should also exist in the metadata registry. For 
example, you put dc.Place in your input forms, you should also add that in 
the metadata registry. You can do that by logging in as the administrator 
and then click the link Metadata under the Registries context menu. Click 
the appropriate metadata schema and then add the new metadata field that 
you included in the input-forms.xml file.
