Hi all,
the uploaded patch will fix all the issues described and add a little 
improvement to the visibility restriction.
Now you can choose more in detail how the field need to be treated out of the 
scope specified (where it has fully visibility and edit capability). You can 
choose to fully hide the field (default behaviour, backward compatible) or only 
show it without allow edit.
To enable this new feature you need to specify the attribute "otherwise" of the 
"visibility" element as "readonly", i.e

  ...
  <visibility otherwise="readonly">workflow</visibility>
</field>
