Good morning, My experience with it has been pretty good. It does replace the bitstreams, as well as the metadata. The structure of the replace folder is the same as the import folder, one directory per item, with contents, metadata and items files. The old bitstreams (and metadata table entries) are all replaced with the new ones.
I haven't checked, but assume the old bitstreams are marked 'deleted' and get cleaned out the next time cleanup is run. We're running 1.4.2, but I imagine the functionality hasn't changed substantially.
