'Dublin core metadata registry' (2) could perhaps be rephrased as  
'Dublin Core metadata schema' which is the one out-the-box schema that  
populates the 'Metadata Schema Registry' (1). Other schemas can then  
be added via the admin interface which gives access to the 'Metadata  
Schema Registry (1)'.

(There is likely to be confusion in the documentation as the metadata  
schema registry was a new addition to DSpace in 1.4. Prior to that,  
only Dublin Core could be used, so the documentation may be  
inconsistent).

sword-metadata.xml is a set of (some new, some duplicate) Dublin Core  
fields that are missing from dublin-core-types.xml and that therefore  
need adding if you want to make use of SWORD. It could be argued that  
this should now be merged into dublin-core-types.xml.
