Add and Remove Selected actions on individual metadata fields have an effect on 
the whole form

In the Describe Item step, each metadata field for which multiple occurences 
are allowed, is enhanced with an "Add" button.
Pressing the add button effectively takes the information, already completed in 
the field, stores it, and makes the text fields empty again.

The problem is that pressing an "add" button for one field, will effectively 
store ALL of the provided values for fields that can have multiple occurences.

Same goes for "Remove Selected". Although each of the metadata fields has a 
separate "Remove Selected" button (making you expect that pressing it will only 
affect that particular metadata field), pressing one of the buttons will 
effectively remove all of the metadata values in the whole form (given the fact 
that their checkbox is marked).

Solution: 
For the Remove selected button: just have one of these buttons on top/bottom of 
the form, or change the behaviour that it only removes the particular field.
For the add button: ensure that it only works on the field it's placed next to.
