This returns internal error message when trying to approve an item in DSpace

This returns the following error message when I click on "approve" an item in
DSpace "An internal error occurred. Please try to redo the process, if the
problem persists, contact the administrators.". What might be happening?
They would know me know which part of the source code may be checking and
generating this exception?
