the <type-based> element is missing
in the input-forms.dtd, which causes the validation of input-forms.xml
to fail if the element is used.
Example:
$ xmllint -noout --dtdvalid input-forms.dtd input-forms.xml
input-forms.xml:41: element field: validity error : Element field content does
not follow the DTD, expecting (dc-schema , dc-element , dc-qualifier? ,
repeatable? , label , input-type , hint , required? , vocabulary? ,
visibility?), got (dc-schema dc-element dc-qualifier repeatable label type-bind

input-forms.xml:47: element type-bind: validity error : No declaration for
