Hello Daniel, The collection settings: DEFAULT_ITEM_READ DEFAULT_BITSTREAM_READ determine the policies for newly ingested items. They serve as a template. During ingest the item policies are derived from these settings. The resulting policies are associated with the item and it's components not the collection.
So check the DEFAULT... settings, to make sure newly ingested items got the proper policies. Then check the policies of items already belonging to that collection. Maybe you had different DEFAULT... setting for that collection at some point. Changing the DEFAULT... settings will not change the policies for already ingested items. In order to mass change the policies for items, use the advanced/wildcard authorization tool. This is available for administrations via the DSpace UI's.
