in a direct answer to your question: it is only possible to define one
input form per collection.
However, DSpace 3.0 came with an extra feature that allows an administrator
to configure fields, depending on the selected type of submission.

In order to use this, you have to make sure the submitter enters dc.type in
one of the earlier pages, and then use the type-bind argument in your
<field> definitions to make sure that this field is only shown for
particular types of submissions.
