I think you just need to remove all anonymous rights, and then it kicks
in. The only caveat (and it tripped me up when I used it) was to
remember to log off before checking to see if it has worked, otherwise
as an admin you are allowed to view all items, so you won't see the
padlock and get the new request page.

Out of the box... No. There were changes in 1.4 to way database calls
are made (the method signatures changed). However I have an up to date
copy I can send to you.
