I have developed a "LinkOut" solution for DSpace Metadata.
You can look at an example of the result at:

(internal links are blue, external links are green)
This was done for DSpace 1.4.1 / 1.4.2.
I can prepare instructions for installation if you are interested.
Please look below at parameters added to dspace.cfg when using our "LinkOut"
class...
